The patient received cycles 1B, 1C, 2A, and 2B of the Linker regimen, and then underwent an allogeneic stem cell transplant. He tolerated the procedure well, and his leukemia remains in complete remission. As of April 2015, nearly 80 percent — 513 of 644 claims for leukemia by veterans who served at Camp Lejeune — were denied by the VA.

In all studies, the doses to the animals were below those causing overt maternal or fetal toxicity. Reported neurobehavioral effects included increased activity, reduced 2-deoxyglucose uptake in the brain, learning deficits, and reduced hippocampal myelin. The effects of PCE exposure during development included the neurobehavioral impairment of rats and mice on certain days of testing, reductions in acetylcholine and dopamine, changes in motor activity and attenuation of habituation, and altered pain and seizure thresholds. These studies of behavioral effects in rats and mice exposed to PCE prenatally or postnatally further suggest that there may be sensitive windows for neurobehavioral impairment during development. Another incidence of water contamination with PCE and TCE was reported in 2002 in the city of Redlands in San Bernardino County, California. Water sample tests and hydrogeological studies from this area indicated that the PCE contamination in the water-supply wells began as early as 1980, with levels ranging from 5 to 98 ppb.

A review of epidemiological studies

For example, in the text of the guidance, deficits in color vision are not mentioned; however, algorithm B specifically lists problems with color vision as one of the few effects that have been identified as a neurobehavioral symptom for the Camp Lejeune program. Performance on a battery of neuropsychological tests was assessed in a small cohort of 35 adults who had been exposed to PCE at Cape Cod in utero or during early childhood between 1969 and 1983 and also in 28 unexposed subjects (Janulewicz et al., 2012). No associations were found between prenatal and early postnatal exposure to PCE and decrements on tests that assess abilities in the domains of omnibus intelligence, academic achievement, and language. The first patient is a 37-year-old Caucasian man who had been relatively healthy until he presented with severe fatigue and muscle ache that persisted for several months. He visited his primary care doctor, and a complete blood count revealed a high leukocyte count and the presence of peripheral blasts.

Panel members discussed these issues, indicating the importance of data discovery to obtain records of PCE use at ABC Cleaners and how ATSDR could account for time frames when data are unavailable. Because the required data types, specificity, and frequency for calibrating water-distribution system models to present-day conditions were unavailable, ATSDR established a field testing program to measure and gather these data. ATSDR obtained data from base EMD staff and water-utility operators on pipeline specifications (e.g., locations, sizes, and materials), storage-tank locations, high-lift pumps and head-discharge curves, operations, production, base housing, and facilities.

Goals of the water-modeling activities are based on the needs of the epidemiologic study. First, groundwater-flow and transport models will simulate the arrival time of contaminants at wells, including the mean concentration and ranges of concentrations during the periods of well operation. Second, the water-distribution system models will simulate the spatial distribution of contaminants by housing location (e.g., Tarawa Terrace and Holcomb Blvd.). Third, groundwater and water-distribution system simulations will be subjected to various levels of analyses to assess and reduce uncertainties. Uncertainties regarding the accuracy of model input arrays and simulation results occur with respect to both groundwater and the model.

SERDP wanted this to be a web-based data management system to disseminate the information from DCERP to a wider audience. During the early stages of DCERP1 and DCERP2, the team met with the base managers who provided a list of challenges of managing natural resources. Some of those challenges included water quality, preserving wetlands, sustaining the use of the training area on Onslow Beach and preserving the terrestrial landscape, where they wanted to obtain their recovery goals for the red-cockaded woodpecker, which is a federal listed species. During DCERP 1, Cunningham said at the symposium, researchers looked at the structure, composition and function of the ecosystems as well as the effect of military training on base lands.

Informed clinical judgment can help identify veterans or family members whose hepatic steatosis may have resulted from exposure to drinking water at Camp Lejeune on the basis of its persistence since residing at Camp Lejeune and the absence of other more likely causes. Because scleroderma onset can occur at any time after exposure to a solvent, any exposed veterans and family members are eligible for health benefits and are accepted to the Camp Lejeune program regardless of when their disease was diagnosed. The committee finds that the guidance and algorithm for scleroderma are reasonable and appropriate.

The clinician then assesses whether it is probable that the chronic kidney disease is attributable to a known cause other than solvent toxicity. If there is no evidence for another cause, chronic kidney disease could be due to toxic exposure. The committee finds that VA’s general approach to renal toxicity in the guidance and in algorithm K is appropriate and in cases of uncertainty with regard to the etiology of the renal injury, the case should be resolved in favor of the veteran or family member. Thus, a patient should not be ineligible for the VA program because of a lack of documented evidence of kidney disease during or shortly after residence at Camp Lejeune. Risks of congenital anomalies in children born to mothers exposed to TCE, PCE, or other solvents during pregnancy have been evaluated in several studies.

It was postulated that the contaminants leached into the water-supply wells from underground water tables, and were subsequently pumped into the water treatment plants where the water was mixed and eventually distributed to both residential and nonresidential portions of the camp. The contaminated wells supplied water to Tarawa Terrace and Hadnot Point, but the Hadnot Point water system was also responsible for distributing water to housing on Holcomb Boulevard until 1972, and then intermittently thereafter (Fig. 1). The contaminated water-supply wells were eventually shut down from February to April 1985. Application of the guidance and algorithm H for hepatic steatosis is challenging because of the high prevalence of other potential causes of hepatic steatosis in the general population.

Sufficient evidence exists to conclude that cardiac defects are causally related to TCE exposure. The bottom line is that I believe ATSDR has done a great job, given the lack of historical data. I’m not convinced that more analysis will significantly reduce the uncertainty.
